We are currently accepting new patients. Thank you for considering Minivasive Pain & Orthopedics.
When you come in for your initial visit, we will ask you to complete our New Patient Registration forms if you were unable to download them in advance to your appointment.
Please remember to bring:
- Insurance card
- Valid photo ID
- Current list of medications

To help expedite the registration process, we have uploaded our New Patient Registration resource forms. Please print and bring completed forms with you to your scheduled appointment.

In an effort to respect the time of all our patients, our staff strives to stay on schedule. For patients who are delayed or arrive late for their appointment, every effort will be made to see you the same day. However, wait times may apply or your appointment will be rescheduled.
PRESCRIPTIONS & RENEWALS
Please request prescription refills and medication authorizations between 9:00AM – 3:00PM, Monday through Friday. Any request made at other times will be processed on the next business day.
